Project Title: Evaluation of process and impact of implementing District-based Mosquitoes Larviciding intervention for malaria Control in Tanzania Entomological monitoring in the control areas

Project Description: Short description of the Project/ConsuItancy/PIatform: The National Malaria Control Program (NMCP) of Tanzania, in collaboration with Towards Elimination of Malaria in Tanzania (TEMT) project is implementing mosquito larviciding in the country to supplement the existing interventions and accelerate malaria control progress towards elimination. Implementation of the intervention has started in three councils of Tanga Region, (Handeni District Council, Lushoto District Council and Tanga City Council, with plans to scale up countrywide. Short description of the Project/ConsuItancy/PIatform: The National Malaria Control Program (NMCP) of Tanzania, in collaboration with Towards Elimination of Malaria in Tanzania (TEMT) project is implementing mosquito larviciding in the country to supplement the existing interventions and accelerate malaria control progress towards elimination. Implementation of the intervention has started in three councils of Tanga Region, (Handeni District Council, Lushoto District Council and Tanga City Council, with plans to scale up countrywide. Understanding of the intervention impact, its implementation cost, cost effectiveness and factors influencing the implementation processes will be critical for guiding scale up. On this regard, the intervention set up has included internal monitoring and evaluation component for collecting data to inform the implementation process and impact. Nevertheless, an external evaluation by an independent party, adds credibility to the information regarding implementation processes, outcomes and impact. Therefore, Ifakara Health Institute is carrying out independent evaluation of the process and impact of the intervention. The main objective of the independent evaluation is to generate information regarding how successful the project activities will be carried out, the factors influencing the implementation processes and the impact of the intervention for guiding subsequent planning and scale up. Specific objectives are: 1. To determine the extent to which the planned activities of the district-based mosquito larviciding have been effectively implemented 2. To determine if the expected mechanism of impact of the larviciding intervention is attained 3. To explore the facilitators and barriers to the implementation of the project 4. To determine entomological impact of implementing the intervention 5. To determine epidemiological impact of the intervention While objectives one, two and three are evaluating the implementation process, objectives four and five are evaluating impact. The ongoing evaluation uses two basic designs: (I) retrospective and prospective data for a longitudinal before-after evaluation, (2) 3 intervention councils versus 3 control councils. Objective five (evaluation of epidemiological impact) will make use of incidence data (reported clinical cases) from the nationally established Health Management Information System (HIMS) through the District Health Information Software 2 (DHIS2). With that system, a large number of control areas can be easily identified and data for them obtained both retrospectively and prospectively. Unfortunately, this is not the case for objective four (evaluation of entomological impact). There is no system established nationally for the collection of entomological data that would be adequate for this evaluation. With TEMT project funds, a system for the collection of the required entomological data (larval and adult mosquito data) has been set-up for the three intervention councils. Unfortunately, the initial funds were insufficient to extend this monitoring to control areas. Therefore, this proposal has received additional funds from the Swiss TPH for collecting entomological data in control areas.
